Bridges to Health—most commonly known as B2H—is designed to recognize that children in foster care can have many caregivers involved in their lives. Children are served in the least restrictive, most home-like setting possible, involving those in their care-giving network. The goal of B2H is to keep children in the community and prevent placement in restrictive settings by offering services to fit a child's unique needs. We aim to reduce hospitalizations and referrals to a higher level of care, keep youth safely in homes and communities and (if eligible) continue services after a child leaves foster case.
